How were two members booked into the same shift?

It is possible for two members to be in a single shift with the status 'SHIFT BOOKED' if within a very short timeframe (several milliseconds), both members accept a broadcasted/shift released shift. As a result, this unavoidable technical issue needs to be resolved by pulling out both members and then booking the correct one in.
